Creatinine is a metabolized from muscle activity. In different individuals,
their muscle total differs, so the creatinine produced may also vary. Children
and elderly people with less muscular amount have a relative lower creatinine
production. Serum creatinine is produced by muscle, but in healthy people a
large part of creatinine will be removed by kidney filters. In this way, serum
creatinine level maintains relative stable. Generally, normal serum creatinine
level in adult females is 0.6-1.2mg/dl, while the range in adult males is
0.7-1.3mg/dl. If one has one kidney, normal serum creatinine level is about
1.8-1.9mg/dl. In some countries, creatinine level is measured by mmol/l. 1mg/dl
equals 88.4mg/dl. In different labs, the reference range may have slight
changes.
Patients have known creatinine level is linked to their kidney functions, but
how are the two numbers interacted? Will you know what is the residual kidney
function analyzed from your serum creatinine vel 4.1?
Generally, creatinine level increases only when kidney function has decreased
by 50 percent. Another important test-glomerular filtration rate(GFR) will be
necessary to determine the exact residual kidney function. In labs, different
formulas are used to measure glomerular filtration rate.
Formula for using creatinine clearance in calculation of GFR is:
Ccr = (Ucr × V)/ Pcr
In the above formula
· Ucr = creatinine concentration in urine
· V = urine flow rate
· Pcr = plasma creatinine concentration
· To calculate the creatinine concentration, urine is collected from the
empty bladder in the morning on two consecutive days with the comparative blood
test.
· To correct the creatinine clearance we use BSA. The formula for correcting
the creatinine clearance is o correct the creatinine clearance we use BSA. The
formula for correcting the creatinine clearance is Ccr-corrected = Ccr × 1.73 /
BSA.
Besides the above, other formulas are available for test kidney function. And
the formulas also take into account age, sex and sometimes weight and ethnicity.
Generally, the higher the blood creatinine level, the lower estimated GFR
is.
When creatinine 4.1 is tested out, the patients are suggested to take active
treatment to prevent kidney progression and repair underlying kidney injury.
Early and proper treatment helps patients avoid entering dialysis.
